Uniform Subscriber Disclosure and Consent Form – Spanish Version Now Available

The Uniform Subscriber Disclosure and Consent Form is now available in Spanish on the Program website and in the program portal.

A watermarked sample of the Spanish language Subscriber Disclosure and Consent Form is available on the program website on the Subscriber Enrollment Requirements & Instructions page.

Subscriber Organizations of selected projects may access a non-watermarked version of the Spanish language form on the Dashboard tab of the program portal. A Spanish version of the guide showing how to fill out the form is also posted in both locations.
