In accordance with a Commission order, the prior versions of the Subscriber Information Disclosure Form and the Utility Customer Consent Form have been combined into a Uniform Subscriber Disclosure and Consent Form.
A watermarked sample of the English language Subscriber Disclosure and Consent Form is available on the program website on the Subscriber Enrollment Requirements & Instructions page.
Subscriber Organizations of selected projects may access a non-watermarked version of the English language form on the Dashboard tab of the program portal. Also posted in both locations is a guide showing how to fill out the form.
A Spanish language version is currently in progress, and an additional announcement will be posted when the Spanish form is available.
Subscriber Organizations and Subscriber Managers must use the updated form moving forward.
