Uniform Subscriber Disclosure & Consent Form Requirements & Instructions

The Commission has adopted a Uniform Subscriber Disclosure & Consent Form (“Disclosure & Consent Form”) to ensure that all customers receive important information about their community solar subscription terms. The Disclosure & Consent Form must include the community solar project the customer will subscribe to.

The Subscriber Organization must provide the customer with the Program Flyer, a complete Disclosure & Consent Form, and a complete Disclosure Form Appendix. The customer must sign the Disclosure & Consent Form before signing the Subscription Agreement.

The Program Flyer, Disclosure & Consent Form, and Appendix must be presented in the subscriber’s preferred language. The program documents are currently available in English and Spanish. Please contact the Program Administrator before translating Program materials into other languages.

Disclosure & Consent Form Generation Instructions

The watermarked Disclosure & Consent Forms available on the program’s website are samples only and will not be accepted. Only Subscriber Organizations with selected community solar projects can access the non-watermarked Disclosure & Consent Form and Appendix.

To generate Disclosure & Consent Forms, follow these steps:

  1. Log in to the Community Solar New Mexico Program Portal using your Subscriber Organization credentials.
  2. Once logged in, navigate to the “Dashboard” section of the portal using the menu links at the top of the screen.
  3. In the “Subscriber Management” section click on “Downloads.” Click “Subscriber Disclosure and Consent Form (English)” or “Subscriber Disclosure and Consent Form (Spanish)” to download the subscriber disclosure package (Program Flyer, Subscriber Disclosure & Consent Form, and Disclosure Form Appendix).
  4. Fill in the Disclosure & Consent Form with Subscriber Information, Subscriber Organization Information, Community Solar Project Information, and Terms and Conditions. For the Terms and Conditions section, summarize the information from the Subscriber Agreement in each box, and list the page number where the subscriber can find the information on that term in the subscription agreement.
