Uniform Subscriber Disclosure & Consent Form
The Program Administrator has updated the Uniform Subscriber Disclosure & Consent Form to incorporate changes to the first paragraph on page 1, and changes to item 11 starting on page 2 of the form. Both of these changes were required by the Commission’s order from November 26, 2024 approving the Uniform Subscriber and Disclosure Consent Form.
Previews of the updated version are available on the program website, and non-watermarked versions are available to Subscriber Organizations with selected projects in the program portal.
Subscriber Organizations and Subscription Managers must use the updated version of the form going forward.
Low-Income Self-Attestation Form
The Program Administrator has created the Customer Confirmation of Income Qualification form that Subscriber Organizations and Subscription Managers can choose to use for low-income customers who are self-attesting to their low-income status.
While it is not a requirement to use the Customer Confirmation of Income Qualification form created by the Program Administrator, if a Subscriber Organization or Subscription Manager chooses to create their own self-attestation form it must be reviewed by the Program Administrator prior to it being used.
The Customer Confirmation of Income Qualification Form is available in a 4-page all-county version that lists the HUD income limits for all counties in New Mexico, as well as 1-page versions for each of the 33 counties in New Mexico. All versions are available in both English and Spanish.
The Program Administrator recommends that Subscriber Organizations and/or Subscription Managers present the Customer Confirmation of Income Qualification form to customers only after asking if the customer qualifies as low-income based on their income, household size, and county of residence.
