Please note that the program administrator, in collaboration with the PRC, has made a minor revision to the Disclosure Form, updating the “[see Note on Page 2]” reference to “[See Note Section]” to accommodate varying page lengths. Both the English and Spanish versions have been updated.
Watermarked versions of the Disclosure Form are posted on the public section of the program website here. Subscriber Organizations of selected projects may access non-watermarked versions available in the Dashboard tab of the program portal. Please use the updated versions of the Disclosure Form moving forward.
