We are excited to inform you that the release of the highly anticipated disclosure form is just around the corner. The form will be made available to Program participants in June. We understand that you have been eagerly awaiting this important document, and we appreciate your patience and compliance with the Program requirements. Please review the following important information:
Subscriber Organization Reporting Deadline Update:
As the disclosure form has not yet been published, no subscriptions could have been executed by the June 1 reporting deadline. Therefore, subscriber reporting for the June 1, 2023 benchmark is not required.
The subscriber reporting functionality will be made available within the portal in June as planned, allowing you to prepare and submit your reports for the July 1 benchmark.
Notice Regarding Subscriptions:
In order to maintain transparency and ensure consistency with program policies, we want to emphasize that no subscription agreements can be offered until subscribers have received the Program approved disclosure form, had sufficient time to read and understand its contents, and signed the disclosure form. This process is designed to provide subscribers with all the necessary information and safeguard their interests before entering any contractual commitments.
Educational Flyer:
In conjunction with the disclosure form, the Program Administrator is working on an informational flyer that will be attached to the disclosure form. This flyer aims to provide subscribers with additional information and key insights about the Program. This educational resource will significantly enhance subscribers’ understanding of the Program, enabling them to make well-informed decisions.
Contact Information Appendix:
We are also developing an appendix to the disclosure form that will enable subscribers to easily identify their subscription manager’s contact information. This appendix will ensure that subscribers have a known point of contact for any queries, concerns, or clarifications related to their subscription contracts.
Subscription Complaints:
We kindly request all individuals who may have been presented with an offer into the program without the proper disclosure form to report the offer to the Program Administrator at [email protected]. As the disclosure form has not yet been released, any offers made prior to its availability violate the program’s guidelines. Reporting such offers will help us address the issue effectively and ensure fair and compliant practices.
